Most breast surgeons are trained in either surgical oncology or cosmetic surgery. I hold board certification in both — through the American College of Surgeons (FACS) and the American Board of Cosmetic Surgery (FAACS) 

This isn't an academic distinction. It changes what is surgically possible for your patient in a single operation.

My approach to breast cancer surgery is oncoplastic by design: the cancer surgery is planned with the aesthetic outcome already in mind, and the reconstructive component is executed by the same surgeon who removed the tumor, with confirmed margins, in the same sitting. There is no handoff. There is no gap between the oncologic team and the reconstructive team. There is one surgeon accountable for the complete result.

Timely access
New patient consultations are available for both urgent oncologic workups and elective reconstructive planning. Patients with newly diagnosed breast cancer are prioritized for prompt scheduling.

Comprehensive surgical planning
Each patient receives a consultation that addresses the full surgical picture — tumor characteristics, reconstructive options, symmetry, and aesthetic goals — so that the surgical plan is complete before the patient enters the operating room, not assembled piecemeal afterward.

Full-spectrum oncoplastic capability
Breast-conserving oncoplastic surgery, nipple-sparing mastectomy, Goldilocks mastectomy, SWIM flap, mastopexy with reconstruction, and ADM-based subpectoral implant reconstruction are all offered in-house. For patients requiring microsurgical autologous reconstruction, Dr. Palakodeti maintains collaborative relationships with microvascular plastic surgery colleagues and coordinates the care transition directly.

Clear communication back to your office
Operative notes, pathology coordination, and post-operative status are communicated back to the referring provider. You will know what was done, what was found, and what comes next.

Breast-Conserving Oncoplastic Surgery
Lumpectomy with immediate tissue reshaping and contour restoration, including volume displacement and volume replacement techniques. Contralateral symmetry procedures performed concurrently when indicated.

Nipple-Sparing Mastectomy with Immediate Reconstruction
Single-stage oncologic and reconstructive surgery where oncologically appropriate. Candidacy evaluated individually based on tumor location, margin distance, and breast anatomy.

Goldilocks Mastectomy
A skin-sparing technique utilizing redundant native breast tissue to provide additional implant coverage without tissue expansion — particularly well-suited for patients with larger, ptotic breasts seeking single-stage reconstruction.

SWIM Flap Reconstruction
Maximizes native skin envelope preservation with complete glandular removal, providing superior coverage and contour for patients who are not nipple-sparing candidates.

Mastopexy with Mastectomy
Simultaneous breast lift and reconstruction for patients with significant ptosis, repositioning the nipple-areolar complex and reshaping the skin envelope in a single operative session.

ADM-Based Subpectoral Implant Reconstruction
Dr. Palakodeti's standard implant-based reconstruction uses acellular dermal matrix in a subpectoral placement, maximizing the patient's own tissue above the implant for superior coverage, reduced complication risk, and a more natural result.

Autologous Reconstruction Coordination
For patients whose anatomy, oncologic picture, or personal preference makes flap-based reconstruction the right choice, Dr. Palakodeti maintains collaborative relationships with microvascular plastic surgery colleagues and manages the care coordination directly — so your patient doesn't fall between teams.

Dr. Palakodeti's dual training in general and cosmetic surgery extends his referral value beyond breast oncology. The following procedures are available for direct referral and can be coordinated alongside or independent of breast surgical care.

Skin Cancer Excision
Surgical excision of melanoma, squamous cell carcinoma, basal cell carcinoma, and other cutaneous malignancies. Margins managed to oncologic standards with attention to the reconstructive plan from the outset.

Skin Cancer Reconstruction
Local flap, advancement flap, and layered closure techniques to restore form and function following skin cancer excision — particularly for defects in aesthetically sensitive areas where standard primary closure is insufficient. Board certification in cosmetic surgery means reconstruction is performed with the same rigor applied to the oncologic resection itself.

Sentinel Lymph Node Biopsy
Performed as part of breast cancer staging and independently for melanoma and other cutaneous malignancies requiring nodal assessment. Integrated into the operative plan at the time of the primary procedure where applicable.

Cosmetic Surgery
A full-scope cosmetic surgery practice focusing on natural, refined, enhancement. Referrals are welcome
